Functional Capacity Assessment (FCA)

Functional capacity reports are comprehensive yet concise assessment reports. They evidence a person's ability to complete functional tasks, what supports might be needed to increase independence and recommendations to build capacity. These assessments are invaluable for accessing the NDIS, plan reviews, changes in circumstance, or funding appeals. Assessment occurs through face to face observation of daily tasks, conversation and reviewing relevant documentation.

Engagement and collaboration with other involved people and completion of standardised assessment tools is also utilised to ensure a holistic approach to assessment and reporting.

Specialist Disability Accommodation (SDA)

SDA reports are completed with those clients who require specialist accommodation to support their goals, which may mean they need robust housing or accessible features in their home. This report will thoroughly evidence eligibility for funding available through the NDIS to strengthen the application process.

SDA will identify level of function across a range of daily activities including personal care and domestic tasks, compare SDA with alternative solutions to evidence value for money consideration, identify required SDA design category and housing type, future planning, considering progression of disability and ensuring recommended support remain suitable.

Supported Independent Living (SIL)

SIL reports will identify level of function across a range of daily activities, giving your support team the insights to make informed choices. Enabling solutions for independent living is always the goal.

This report will recommend ratio of support needs and intensity levels to ensure appropriate funding, compare SIL with alternative solutions to evidence value for money considerations, future planning, considering progression of disability and ensuring recommended support remain suitable. 

Home Modifications (HM)

Modifications to the home, whether minor or complex, can greatly impact a client's overall quality of life and help maximise their independence, assisting them to achieve participation in everyday occupations to lead healthy and independent lives.

The aim of the assessment is to facilitate our clients to do what they need and want to do, in the most effective and safe manner, within their home environment. We provide a personalised service and approach, working directly with our clients to empower them throughout the whole process.

Equipment Needs - Assistive Technology (AT)

The right AT equipment can greatly improve a person's quality of life. There is no one-size-fits-all solution when it comes to equipment needs, so our team will take the time to understand the individual needs of our clients, assessing beyond just the physical.

Our approach considers such things as environment, the location where you need to use it, your lifestyle and the things you want to do and enjoy doing, for example. Our goal is to identify the best equipment solution to help clients gain independence in daily tasks. 

Skill Building & Development

We support clients through ongoing sessions to do the activities they need and want to do (performance), and to facilitate engagement in their life roles and situations (participation) within their everyday lives and natural environments.

Your OT will conduct evidence-based assessments and will develop solutions and strategies that are relevant and tailored to you and your goals, with the view to improve engagement, develop and build skills, and increase independence.
